Please join us for a ribbon cutting event for Community Support Advocates, Inc. celebrating the grand opening of their Integrated Health Program. They will have an open house on Wednesday, April 2nd from 4-6 pm with ribbon cutting ceremony at 4:15 pm. They are located at 6000 Aurora Ave, Ste B in Urbandale.
CSA’s Integrated Health Program provides whole health coordinated care for young people, age 16 to 26, with a serious mental illness or serious emotional disturbance who are Medicaid Eligible. This program is designed to address the unique mental health, physical and social needs of young people and their families as they transition into adulthood.
